Describe your field placement, including what kind of organization it is, what population the organization serves and what services are available, and/or what work the organization does. Please include if the organization is for-profit, non-profit, public and its funding source. Include the mission statement of the organization. If you are interning for a smaller organization that fits under the umbrella of a larger organization, you may need to use the mission statement of the larger organization if there isn’t one for the smaller organization. If you are unable to locate a mission statement, note that in your description. Samaritan Daytop Village is a non-profit human services organization with multiple locations that serves an overlapping population by offering a variety of programs and services to individuals and families in need. Rehabilitation, supported housing, integrated healthcare, preventive services, and early intervention are examples of such services. My placement is with a Samaritan Village host named Independence. The Independence outpatient program is located in the Bronx at 510 Courtlandt Ave. Independence is an integrated behavioral health and substance abuse treatment program that offers counseling, medication management, case management, and peer support. It stresses relapse prevention, a healthy lifestyle, and personal responsibility in order to equip individuals with the skills and support they need to live a sober and independent life. Assessment and Exploration Functional Categories Indicate the degree to which each factor is present in your organization. Organizational Planning Summary and Analysis The Samaritan Village Independence vision and goals, basic values, and dedication to working with and cooperating with the community are all outlined in the strategic plan. Under the strategic plan, for example, steps are taken to design a complete and comprehensive assessment process, find and train employees, and use evidence-based practice. The Samaritan Village Independence Outpatient Program has a clear mission: to offer those in need high- quality, comprehensive mental health and substance abuse
3 treatment services. The purpose of Samaritan Village Independence is to provide quality and compassionate care to persons in need of behavioral health (substance treatment and mental health) and social assistance. The mission statement provides the basis for the strategies, goals, and objectives. These goals and objectives are complete and clear, concentrating on creative programs that assist and advocate for clients and families to achieve genuine and holistic self-sufficiency. Samaritan Village Independence objectives reflect and measure client outcomes in order to ensure that the organization is meeting its mission and providing the best possible care to those in need. These outcomes include improvements in client well-being, increased access to services, and improved quality of life. The plans of Samaritan Village Independence are used on a regular basis to ensure that the organization is meeting the needs of its clients and providing them with the services and resources they need to improve their quality of life. Management of the Environment Rating To What Degree 7. 4 Can you identify the problems or needs the organization intends to address? 8. 4 Can you define the target population (e.g., demographics, geographic boundaries) the organization intends to serve? 9. 4 Are key stakeholders (funders, other agencies, regulators) satisfied with agency programs and services? Management and Environment Summary and Analysis Independence seeks to assist and support individuals experiencing substance abuse, mental health disorders, and other issues, while also enhancing their quality of life. The target population that the Independence outpatient clinic intends to serve is individuals and families in need, including those living in poverty, those living with mental illness, and those facing other challenges in their lives. Key stakeholders such as OASAS, SAMHSA, Friends of Recovery, Manatus Development Group, and other community
4 partners are generally satisfied with the services and programs provided by Independence Outpatient.

Client Relations Summary and Analysis Independence Outpatient participants typically believe that what they require is immediate access to high-quality care, complete treatment options, and assistance from qualified, experienced, and compassionate interdisciplinary staff. The Independence Outpatient Clinic's policies and procedures clarify patient expectations and guarantee that services satisfy their needs. It also lists appointment times, privacy policies, and client expectations for communication and collaboration. The clinic offers customers programs, services, and resources. Clients generally report a high level of satisfaction with the clinic's services. Part I submission, due Module 4, ends here.
