Hazards: Propane tanks right next to each other. The valve was stuck. Propane gas escaping. Properties: Tank Never Changed. Inexperienced Technician doing the job alone. Propane is a clear vapor. Junor Tech waited far too long to report. Identify the factors or conditions necessary for the hazard(s) to transition to the mishap. These were referred to as Initiating Mechanisms (IMs) in the text. Training is not required in many states, there were no safety procedures for this kind of case. Everyone involved with this was unsure on what to do in the event of a propane leak, leading to this issue getting worse and worse as time went on. Identify as many Targets that are the result of this mishap. Several people were targets, 2 being the working technicians.
