FINAL MILESTONE 2 Finale Milestone: EHR Recommendations for Alpha Corporation Alpha Corporation was in need of an analysis of their current system which was a combination of paper and electronic system. This is a full analysis from beginning to end of what is recommended from analysis of the corporation itself and its needs to the system itself that is recommended and then what is needed after implementation of the system. Analysis: Organizational Overview: Organization Alpha is a 150-bed hospital, with five designated intensive care beds. It has an emergency room designated as a level III trauma center. Currently this healthcare organization is utilizing a hybrid system of paper documentation and electronic scheduling. They have multiple services that the organization provides as well. The goal for this organization is to create a solution for an EHR that will be able to share information between all of its services and be user-friendly so that their current long term staff will have an easy time learning it. This will allow them to forgo the paper documentation and implement an electronic solution that will communicate with each other and help the locations be connected better. Analysis: Organizational Overview: Services Alpha’s designated care areas are for outpatient surgery, orthopedic surgery, women’s health, pediatrics, and cardiology. Along with these designated care areas they provide some auxiliary service lines which include five family practice clinics, two internal medicine clinics, one cardiology clinic, one neurology clinic, one orthopedic and sports medicine clinic, one eye care clinic, one women’s health clinic, and one pediatric clinic. They also provide outpatient rehabilitation services and a community wellness program.
FINAL MILESTONE 3 Analysis: Organizational Overview: Stakeholders The stakeholders you would want to consider and make an important part of the selection of the EHR would help not only make an appropriate decision for the company it would help with a more detailed view of the organizational needs. The key stakeholders you would consider are the following: Providers: Having the collective input from your practicing providers (physicians, nurse practitioners, physical therapists, etc.) is an important part of the planning and selection process. They are the end user of the program and rely on a system for efficient and accurate patient care. Front office staff manager or lead: This role will provide insight on how well the choice of EHR system will be for administration work needing to be implemented such as demographic input, scheduling, entering insurance information. Their role can also help decide on user-friendliness of the program as well. Biller and Coder: You would want someone from this role to speak on the EHR choice because if the billing system doesn’t run correctly or communicate to them in an efficient and timely manner then they cannot do their job to submit to the payers. Reimbursement depends on accurate and timely coding. Administrator: An administrator from the company should be present for the planning and decision making process to lend insight into the financial capabilities of the company. They would also be able to help guide for a broad vision of what the company needs and make sure all decisions on an EHR fall in line with appropriate guidelines and laws. Lab Lead and Pharmacy Lead: These roles would help to determine if the ordering system for the lab tests and medications were user friendly and appropriate.
FINAL MILESTONE 4 IT Specialist Lead: The IT department should have a hand in the planning and decision making as well. They would be the ones to implement the program across all departments. They could speak to the capabilities that the company may already have and what it would need to fill the deficiencies. This would help the administrator to plan for any extra cost on top of the EHR itself. Design: Applications: Health Informatics According to Sayles, et al (2021) health informatics use the data that is generated by the health data analytic techniques to make decisions that impact the processes, technology, and people who must collaborate to make better decisions for the benefit of the target population. When thinking about all the informatics systems that could be utilized to make a large healthcare company like Alpha function properly, I would suggest the following: Registration System: This system would not only handle registration but would handle any admissions, transferring of patients, discharges whether it is a new patient or returning patient. It would collect the demographic information from name, address, phone number up to and including the insurance information for billing purposes. Billing or Financial System: This system would interface with the Registration system to receive the insurance information, any copays that are associated with the patients, processes claims, prepares the patients bills according to what insurance doesn’t cover. Point of Care Charting: This allows for structured and unstructured data fields for easy communication between providers, nurses, care teams, billing teams and allows for quick return of results to give to the patient. Provider-Patient Portal System: This will streamline communication between the provider, the care team, and the patient. Provides secure messaging capabilities to allow
FINAL MILESTONE 5 for discussion about their health information. It would have to be HIPPA and ePHI compliant to use. Ordering Communication System: This would allow for orders whether it is blood tests, imaging, or pathology to be placed electronically and the results to return in the same system for the provider and the care team to review in a prompt and efficient manner. Electronic Prescription Record: This system allows for prescriptions to be sent to pharmacies electronically, refill requests to be received electronically, monitoring of controlled substances. This system will also help with monitoring refill dates, start dates and who prescribed the medication. Document Management System: This system will interface with the EHR to allow for any paper records coming from the outside to be scanned and managed and linked to the correct patient. Data mining system: This system may not be part of the EHR directly but one should be put in place and an EHR found that would allow for a program to be used. This will help with any quality measures, trends, or research to be done if needed. Design: Applications: Requirements The requirements that Alpha would need to employ would be to make sure that their EHR follows all HIPPA, ePHI, and HITECH policies and laws. This would include making sure that all covered entities protect the patients’ protected health information (PHI). They would also need to make sure that they are having patients sign an informed consent for HIPPA, a consent to treat which would include language based on being able to share their information for referral and laboratory purposes.
