Assignment 11.2 1. A patient comes into the office for treatment. He does not return because he is dissatisfied with Dr. Practon’s treatment. Is it necessary to keep his records when he obviously will not return? Why? Yes, health records must be kept as required by federal and state law for a certain period. If the off is a research office, it should be kept permanently. It is recommended that records of a dissatisfied patient be kept indefinitely because of the possibility of a lawsuit later. 2. A coding specialist is abstracting information from a provider’s office note. Information in the exam conflicts with information in the both the history and the final impressions documented. What should the coding specialist do? A billing and coding specialist is reviewing provider notes to complete a claim. They need clarification on whether the procedure performed was on the left side, right side, or bilaterally. The specialist queries the provider, and the provider confirms it was a bilateral procedure.
