2 Mr. Lessley has tasked me with investigating some operations and tasks on the worksite that could have significant potential for injury, these are my findings and recommendations for the following hazards. Operating an electric forklift in narrow aisles: Hazards involved are hitting the shelves of the aisle and pedestrians. Let’s start with hitting the shelves with the forklift and break this down by using the hierarchy of controls. Elimination: We could move the aisles further apart in the warehouse to create wider aisles eliminating the hazard of working in tight spaces. Substitution: We could use a narrow aisle forklift, also known as a reach truck, which was designed to work in tight spaces. (2021) Engineering control: automatic braking system that will stop the forklift if the operator is about to hit something. Administrative controls: When the operator goes through the forklift training, we train them to only operate the forklift if a spotter is present. If need be, more than one spotter may be utilized to ensure safe operation. PPE (Personal Protective Equipment): I did not think that use of any specific PPE in this scenario would help prevent the hazard. After looking over each control measure, my recommendation would be to make sure we are using a narrow aisle forklift, reach truck, along with a spotter to prevent from running into the aisles. I find this substitution to be the most effective way to eliminate the hazard of hitting an aisle with the forklift because we are now using the right equipment for the job and doubling down by having a spotter present as well. Now, let’s look at the hazard of pedestrians.
3 Pedestrians could be struck by or even killed by a forklift, let’s assess the hierarchy of controls to help avoid this hazard. Elimination: Do not allow pedestrians in the forklift area while forklifts are being operated. Substitution: Put in walkways for the pedestrians, this will help them avoid the path of the forklift. Engineering Controls: Install barriers around walkways to prevent the forklift from hitting someone. Administrative Controls: Train the employees not to enter the warehouse while the forklift is being operated. PPE: Have pedestrians wear a reflective vest if they must walk through the warehouse. My recommendation for this hazard would be to install barriers around the walkways to prevent the forklift from hitting someone. This allows pedestrians to be able to walk through the warehouse safely while being protected by the barriers and staying on the walkways. The second task we will are look at is repairing the hydraulic system on the gas-powered forklift: Hazards involved in this task are being injected with hydraulic fluid and being struck by pressurized hoses. Let’s look at the hazard being injected with hydraulic fluid while making repairs. Elimination: Have a professional certified mechanic perform all maintenance on forklifts Substitution: I researched to see if forklifts used pneumatic lifting systems and they do not. I don’t believe there is a substitution for this hazard.
