Module 12: Otorhinolaryngology - Report 3: Operative Report, Pressure Equalization Tube Removal Rationale Report Professional Services: 69424-50 (Removal, Tube, Ear, Middle) ICD-10-CM: T85.698A (Complication, prosthetic device or implant, specified, mechanical, obstruction) RATIONALE: The patient had tubes placed in her ears and now has been experiencing complications due to blockage of the tubes. The decision for surgery was made and the service is performed for bilateral removal of ventilating tubes under anesthesia, 69424. The modifier -50 is appended to show that this was performed bilaterally. The removal of PE tubes (pressure equalizing) is included in the insertion of the PE tubes unless general anesthesia is required. ICD-10-CM: The diagnoses are listed in the surgical Preoperative section of the report as obstructed PE tubes. This is a complication. The blockage of the PE tubes is considered a mechanical complication, reported with T85.698A. The seventh character "A" indicates the initial encounter. The report states that the patient has had recurrent infections and persistent fluid; however, it is incorrect to report chronic otitis media, as the report does not state it is a current condition.
