On 1/12/2017 Mark Kopolovicz came to the staff office. Mr. Kopolovicz appeared to be alert and groomed. He express to staff that he finally got a haircut. Staff was excited to see Mr. Kopolovicz cut his hair since he express that he was enjoying the new look. As Mr. Kopolovicz was sitting with staff discussing how his day was going JASA Social Worker Jessica Alfaro came into the staff office. She came to Genesis to meet with Mr. Kopolovicz to discuss his rental arrears and hoarding. Ms. Alfaro came to Genesis two previous time to see Mr. Kopolovicz 12/27/2016 and 12/29/2016 to provide him with services to receive a one-shot deal in hopes to keep him housed. Mr. Kopolovicz continue to project all attention off of him. As SUS Staff is trying to get Mr. Kopolovicz to understand the significant of his current situation he continue to ignore what SUS Staff and JASA Staff is saying. Ms. Alfaro informed Mr. Kopolovicz that he can gain services from JASA that will provided him with financial management. Mr. Kopolovicz continue to “self-talk”. Wellness Coach Teresa Cain and JASA Social Worker Ms. Alfaro step out the staff office and set up an appointment therefore a conference can be held for Mr. Kopolovicz on February 7, 2017. SSA received a phone call from Verna Goecke with PRPV. Verna shared that it is an ongoing daily situation in which Kaitlyn provokes another resident (KN) and starts making accusations against the resident. She shared that daily, Kaitlyn will go into KN’s room provoke her which triggers KN who then starts yelling and screaming. When Kaitlyn has KN yelling and screaming, she calls her mother and has her mother listen to KN yelling in the background and informs her mother that KN is targeting her. Verna shared that on the previous evening; Kaitlyn called her mother and informed her she had an ear infection. Staff reported to Verna that the Kaitlyn came out of her room with her mom on speaker phone and Karen Kohn was yelling at staff through the phone telling them they were “neglectful” and “abusing” her daughter.
Pt. requested to meet with this writer for a brief session. Pt. informed this writer that she needs to transfer to Connections CSP Inc. because she currently can't afford pay for the AMS of DE TX services. Pt. expressed feeling very concern about her transportation issues and needing to fix her car. Counselor told Pt. that this writer is going to help her with the process but she needs to talk to the AMS Director prior his appointment with Connections program. Counselor encouraged Pt. to obtain information about the Connections program requirements and rules, to pay current AMS balance and to discuss what she learned from this TX. Service Coordinator (SC), Jennifer Stoker met face to face with provider, Aiesha Crayton and consumer Jonathan at his group home. Jonathan informed SC he has change his mind he don’t want his own apartment. SC asked why he changed his mind. Jonathan noted Aiesha told him that she couldn’t pay a staff member to live in his apartment with him. He noted Aiesha could be his provided but it will not be 24 hour care for him at his apartment. Aiesha noted she also talk with him about the amount of money he would have to pay for the apartment. Aiesha noted the apartment he want was 700 dollars. Jonathan noted he check would cover the rent but he would have money to pay his other bills. Jonathan noted he really was not going to move out. He was
Mr. Gotcher recently moved to another Ambitions home due to community concerns. This elevated Mr. Gotcher’s stress and anxiety because he feared being homeless despite being told that DDA was working to find him another home. It should be noted that Mr. Gotcher at no fault for this move. It was reported that a housemate disclosed to a new neighbor that the clients living at the home were Community Protection clients.
CM was on vacation for the period of 11/25/2016 to 12/12/2016. On 12/17/2016, CM met with the client to complete Bi-weekly ILP Review and to obtain information concerning TPS (“Temporary Protective Status”). Client was dressed appropriately for the weather. She was well-mannered and groomed. In the meeting client appears to be cooperative, friendly and forthcoming with information. Client speech was spontaneous and soft. During the meeting client become emotionally teary when CM inquiries about client TPS status. Client provided CM with a brief history how she came into the shelter system. Client continues to report her most important means of support passed away recently, which is part of the reason she came into shelter. Client continues to denied suicidal or homicidal ideation.
Client Contact: Ms. Steed came into the office to schedule a HV (Home Visit). The writer introduced herself to the client as her new Housing Specialist. The Housing Specialist then asked Ms. Steed when would she like to meet and she said that she would be available on Monday, June 6, 2016 at 9:30am. The client reported that she has just been hired at Barnes Jews Hospital in the housekeeping department. Ms. Steed said that she was very excited to start her new job. She also stated that she was interested in moving into a one or two bedroom apartment due to her sisters moving out of their three bedroom apartment. She expressed that the three bedroom apartment was too expense for just her to maintain. I told her that I would look into her possibly moving into a one or two bedroom apartment. I asked if there were any repair issues in her current apartment she stated that she didn’t have any home repair concerns at this time.
Throughout the course of 6/22/15- 6/27/ Igoe REACH has come into the home of Jessica to observe her behavior. The behaviors, which Jessica engages in, place her in harm of herself and her peers. Monday she engaged in elopement, which then led to a sit down strike in the middle of the road. The incident required two staff to physically escort her in the home while trying to deescalate the behavior, which included screaming and kicking staff, was punched in the face another staff was also forced in a ditch that was filled with mud. All these incidents happened while in the presence of her support coordinator for Prince William County. Collin another in home counselor came to observe Jessica. I made contact with him on his second time with Jessica. Collin was unaware of Jessica normal behavior so staff shared with him that he should, “observe” her getting of the bus. Immediately entering the house she started to engage in behaviors that led staff to verbal redirection. This took place for about 45 minutes until the AD for Community Concepts (Regina Goldston) advised him to back away and observe from a distance. It has been noted that Jessica will engage in more behavior with an increase presence of staff. Collin spoke with the home manager about the normal behaviors. The OM asked what exactly could reach provide to Jessica as far as support goes. Met with the client and his mother on this day. Pedro appears to be healthy and safe according to his age. The client resides in a two bedroom, one bathroom apartment with both parents, and two brothers ages 4 years and 6 years old. The house appears to be well maintain. MOB indicated that they recently moved to the apartment looking for affordable rent. They have moved over two times in the last year. As per MOB the primary source of income is from the FOB''s employment as a migrant worker. MOB indicated that the family dynamic is positive and denied past or present abuse or domestic violence. She also denied any idea, intent or attempt to commit suicide past, present or future.
I: CM used open ended questions to inquire about the client’s missed appointments with LAMP VASH. CM questioned client regarding updates with his social security payment. CM discussed a plan with the client that will help the client prioritized all of his housing appointments. CM explained to the client the importance of keeping all of his housing options available by attending all of his appointments and meeting all of the requirements.
Writer met with Joselina to review her care plan and completed it with her signature. Joselina also signed her crisis plan that we completed on the phone. Joselina told care manager that she is now linked with a new counselor (Andrew Irish) at Lake Shore Behavior at Abbott Road. She said she likes her new counselor because she really listens to her and understands her as compared to her old counselor. Joselina said her next appointment with her counselor will be on the 6th of next this month. Joselina said she will discuss her social anxiety with her counselor and hopefully she can help her face it. On Monday, October 3, 2016 Hud Vash worker Frank Ansah had a scheduled appointment with Anthony Rizzotti. The meeting was to discuss Mr. Rizzotti’ rental arrears and benefits. Mr. Frank asked Wellness Coach to provide a copy of Mr. Rizzotti’ Social Security and residential ledger. When Mr. Frank did not come to Genesis II on October 3, 2016. The next day (October 4, 2016) Wellness Coach contacted Hud Vash Cash Worker Mr. Frank to follow up on the appointment he was to have with Mr. Rizzotti. Mr. Frank informed Wellness Coach that Mr. Rizzotti called and reschedule due to having an appointment at the VA Hospital. The next appointment Mr. Rizzotti has with his Hud Vash case worker is to be for October 7, 2016.
On 7/15/2015, CM conducted a search via Security to locate client. Client was found sitting in the Cafeteria. Client was asked by Security to come upstairs to meet with her assigned CM met with the client and completed Bi-Weekly ILP Review update information concerning NY Psychotherapy Mental Counseling referrals. Client replies “she probably had a minor seizures attack because she doesn’t remember speaking with CM.” CM also inquires about the numerous referrals to NY Psychotherapy Mental Counseling for individual session to address client depression. On 7/18/2015, client came to the social service office complaining that she being harasses by residents in the Kitchen. Client mentioned while she was having lunch. One resident tap her on her back with the cane. She continues to mention residents W. Wright got involved and told client that she have “Her son is her custody”. Client was upset. CM went to the Kitchen to investigate the allegation. CM was informed by residents W. Wright that the client had her cane in the way and she was asked to move the cane. Michael S: Client was eager to declare he will be making a $100 Aftercare payment on Friday and is attempting to learn how to budget adding he figure it’s about time. On the topic of Intimate Relations the mother of the client’s child, Lilly, has had the most dramatic influence on him personally. Admitting it is a sore subject for him, he explains that he used to be angry that she was unsure who Lilly’s father was initially, but now he defends her due to her dysfunctional upbringing and has an amicable relationship with her to this day.
Golf shoes neatly stacked on shelves along two walls of the tan room, shreds of grass sparced all over the black mat on the floor and the smell of shoe products lingering in the air, Mr. David Doak took a seat on a worn out wooden stool. Resting one foot on the ground and the other on the stool, he fixed his posture and his blue eyes looked firmly into the black watch on his wrinkled wrist, proceeding to say, “I am a couple of months shy of 66.” Doak is a nonchalant person that enjoyed teaching to the point of caring deeply about his job, setting higher standards for his students and appreciating equality between male and female workers.
