Within the context of the proposed DNP project, several organizational barriers impact the design, implementation and the effectiveness of the project. By identifying and addressing each barrier the project can continue to positively impact the quality, safety and improved health of children with special healthcare needs. At the Duke Transition Clinic, the two main barriers to the design and implementation of the project are the diverse patient population combined with staff expectations and the culture and standards of clinic operations.
Clinic Standards of Operation An organizational issue that could potentially impact the implementation of the project is the structure of patient visits within the transition clinic. The transition program is funded by and conducted in partnership with a teaching hospital; therefore, the location of the clinic changes on an almost weekly basis and has only one staff member who is solely devoted to the program. As the program is in conjunction with a teaching hospital, the clinic is staffed primarily by residents supervised by two attending physicians. Consequentially, the flow and structure of the clinic varies by week as the residents staffing the clinic are rotated on a bi-weekly basis.
The variation in clinic location and physicians causes disorganized and inconstant transitional care delivery. Within the context of the proposed DNP project, this problem has the potential to make data collection and the creation of realistic

Physical distribution – Services are administered through care givers and physicians during visits to either the hospital or office visits. The implementation of a combination of evidenced-based models such as traditional patient navigation, and the wrap-around services model, coined by the project, Therapeutic Patient Navigation is now utilized to address the complex long term needs of the population that we serve.
Planning tools are vital aspects that assist team members in projects to brainstorm ideas as well as organize information achieve the visions of the organization. Implementation of the tools of planning in the Integration care delivery protocols in the Intermountain Health Care was a sure way of improving the patient care in the facility. The tool that was applied in the case was evidence based that was believed to achieve better results (Geertman, & Stillwell, 2009). Opposition from the nurse’s quarter faced the project where some nurses were of the view that the different patients are unique and therefore unique services should be offered to them.

The nurses will need to be onboard and understand that this process change will help us give our patients the knowledge they need as well as increasing their satisfaction. Each nurse will be responsible for maintaining the educational materials while making sure the information is accessible to our patients. An important part of this process change is to make sure the staff is onboard and the patients are receiving the information.
