Overview of the community clinical site My clinical site for community health was at Kansas City rescue mission located at 1520 Cherry St, Kansas City, Missouri. I started from August 30th to September 14th. The site is a homeless shelter rescue facility for men. The shelter provides the following services: meals, showers, healthcare, laundry services, bible study, drug and alcohol addiction support groups, and medication administration. The healthcare service at Kansas City rescue mission is setup like a doctor’s office. The clients sign up to be seen by the nurse and once they come into the nurse’s office, they are interviewed, a focused assessment is performed, and recommendations are determined. The shelter also provided assistance in job placement to men that resided in this shelter and place to stay for those with no alternative shelter. Involvement with the Agency I was in interviewing and charting the subjective data, performed a focused assessment based on what the client came in for, made recommendations, provided education, and gave over the counter medication to the client under the supervision of my preceptor. While interacting with the men at the facility, I felt accepted and appreciated. For example, during visits at the nurse 's office, most of the men would open up and share their stories and things that they are struggling with emotionally and socially, even though I hadn’t known them long enough to establish a relationship and trust for that kind of
My community clinical was at the Denver House Mental Health Association located in Tulsa, Oklahoma. This is a day center for targeted for the people with mental illnesses and homeless, which opened in 2012 (mhaok.org). Many of the homeless population in Tulsa, Ok suffer from a mental illness. Due to the lack of resources in our community, those who suffer from a mental illness end up on the streets. The day center provides a safe warm place for the homeless and those with mental illnesses. They provide a stigma free welcoming environment were those feel welcome to come and interact with peers. The organization also has specific workers that help with finding shelter, apartments, and jobs. They also have a community closet for clothing that is open every Tuesday and Thursday. You must be 18 years old, be drug free while on premises, and follow basic rules to come to the Denver House.

Over the year both positive and negative influences have influenced my learning. The skill I have developed for learning is my aspiration is to become a midwife, this influences my learning in the way that it gives me the hunger to learn new things and improve my knowledge and grades. By improving my knowledge I will be able to go onto the next stage of my development. In the United States we are very fortunate. For the most part we do not live in fear of disease outbreaks from day to day. Our government along with the Centers for Disease Control and Public Health Departments have managed to have adults and children vaccinated and set up recommendations regarding those vaccinations. Despite all that is done our country allows freedoms. One of these freedoms is to not vaccinate your child from communicable diseases due to personal beliefs. In some instances even just two independent cases can lead to an outbreak, eventually leading to an epidemic, or pandemic which refers to a global outbreak. Measles is a communicable disease that is highly contagious according to the World Health Organization (WHO). In early April 2013 a measles outbreak was discovered in North Carolina. By mid-May the outbreak had been identified in Stokes and Orange Counties via 23 active cases. Every case was linked back to a family that had spent 3 months in India and had not been vaccinated. By the 16th of April the state laboratory of Public Health was able to confirm the diagnosis, with the last known case being confirmed on May 7th. The investigation of this outbreak revealed 4 patients with a confirmed diagnosis that had received one of vaccination of the two part series. The other 19 cases had not ever been vaccinated.

A community health assessment is a fundamental instrument of public health practice. Its objective is to depict the health of the community, by presenting information on health standing, community health needs, resources, and epidemiologic and other studies of present local health problems. It seeks to recognize target populations that may be at augmented risk of poor health results and to increase a better understanding of their needs, as well as evaluates the larger community surroundings and how it relate to the health of people. It also identifies those areas where better information is desired, particularly information on health differences amid different subpopulations, quality of health care, and the incidence and severity of disabilities in the population. The Community Health Assessment is the foundation for all local public health development, giving the local health component the instance to recognize and network with key community leaders, businesses and concerned residents about health priorities and concerns. This information shapes the foundation of improving the health status of the community by way of a strategic plan (The Municipal Public Health Services Plan Community Health Assessment Guidance and Format, n.d.).
The general condition of the homes in Rockport is diverse. Many of the homes are multi-million dollar structures while others are dilapidated and in need of serious repair. People seen in the downtown area are walking but away from that area, most people are travelling in cars. Some people are travelling on bicycles or golf carts in designated areas. Three areas of public housing are noted but there are no visible signs of public transportation. Numerous campaign signs are visible throughout the entire community. One adult day care and two child day centers are noted during the survey. Two grade schools, one middle school, two private church schools, and one high school are seen during the survey.
