Case summary:
A patient, age 10 years diagnosed earlier with Wilson's disease, reported with giardiasis at the emergency room.
Characters in the case:
Richard Brennan, the patient under evaluation.
Adequate information:
The patient was being treated with penicillamine for Wilson's disease. During the treatment, he has reported of multiple sinus infections, one instance of pneumonia and now giardiasis. Blood investigations reveal normal B and T cell counts, normal IgG and IgM but very low IgA compared to reference levels. His case was assessed as that of the drug-induced transient form of IgA deficiency attributed to the use of penicillamine in Wilson's disease.
To Determine:
The nature of antibodies that use the same transport receptors as dimeric IgA that would have been present in the lumen of GI tract and mucosal secretions of Richard while he was taking penicillamine.
