Concept explainers
To explain: The outcomes that might have occurred if there had been failures or malfunctions in parts that were shipped to company S.
Case summary: In the given case, federal authorities accused a worker of a firm in the state of NY of forging signatures on inspection reports. An employee in question was a manufacturer in the company PM which specialized in high-tolerance machining to build space flight vehicles for company S and other contractors. The employee worked in the company PM as a quality assurance engineer and allegedly falsified several inspection reports by saying that the parts had been inspected but in actuality, they had not been inspected. If any part which is not inspected includes malfunction, then it could result in catastrophic failure of the company’s mission. When the employee was arrested by the authority, S also terminated its contract with the company PM. Therefore, the company ceased its operations which makes many workers jobless.

- Read and analyze the following article. Manila Bay Beach Nourishment Project After years of being known for its polluted waters, Manila Bay suddenly got a facelift as the government decided to take a big risk by overlaying mounds of artificial white sand (dolomite) on a part of the bay's naturally gray shoreline. This project, which is spearheaded by the Department of Environment and Natural Resources (DENR), was instantly met with harsh disapproval. In September, the overlaying of the dolomite in Manila Bay beach or the beach nourishment project caught public attention, stirring curiosity and setting off waves of people flocking to the area to get a glimpse of the "white sands". Many criticized the project, throwing allegations at the DENR, calling the project a health hazard and a waste of public funds. But the DENR emphasized that the beach nourishment with the use of dolomite is a significant component of the rehabilitation aimed to protect the coastal resources in the area and…arrow_forwardCan the identification of IT risk be left to the care of external consultants only without theinput of permanent/ full time IT employees with the company?arrow_forwardIt was a clear case of structural failure. It was critical to understand exactly what had happened. The incident raised many questions; it was not just that the walkway fell or even that a miscalculation was evidently made, but also how every contributing factor came to exist and why errors were not caught earlier. The disaster in Kansas City occurred due to a connection that was overloaded because of an ill-advised change to a badly defined structural detail. The design failed to meet the requirements of the Kansas City Building Code. Checking the calculations at the design stage could have prevented this disaster. Investigations reveal that neither the designers nor the builders assumed responsibility for the final product. The design review process was ineffective. The accident was caused by relying on a longitudinal weld that wasn’t strong enough. For the engineering profession, the walkway collapse tragedy became a classic model in studying engineering ethics and errors. The…arrow_forward

The Mid-West Enterprises currently sells a product consisting of three components arranged in series (i. e., all three components needs to be working in good condition) with the reliability of each component as shown in Figure 1 below. However, they want to improve the reliability of their product and are considering two alternate designs that mix the series-parallel configurations with the reliability of each component shown in Figures 2 and 3 below. Ignoring the cost and all other aspects, the management of Mid-West Enterprises has asked you to analyze each design option. Therefore, please answer the following questions clearly stating the logic and calculations needed. What is the reliability of each design in Figures 1 through 3 above?arrow_forward
