54-Year-Old Female with a Defective Heart Valve
Dr. El-Sheikh is a 54-year-old Egyptian female who complains of weakness, fatigue, shortness of breath, chest pain, and dizziness. Her heart rate is 96 beats/min, and her blood pressure is 148/54 mm Hg. You notice that she nods her head with every heartbeat, and her radial pulse is very strong and abrupt. Her heart sounds are abnormal, with a long diastolic murmur at the right sternal margin in the second intercostal space.
Her doctor orders an echocardiogram, which shows that Dr. El-Sheikh has severe aortic regurgitation (backflow of blood through an incompetent valve) and an enlarged left ventricle.
5. The additional muscle in Dr. El-Sheikh's enlarged left ventricle means that it requires more oxygen. Which arteries are most important for supplying this ventricle?

- High blood pressure would be a result of ___________ . a high cardiac output and high peripheral resistance a high cardiac output and low peripheral resistance a low cardiac output and high peripheral resistance a low cardiac output and low peripheral resistancearrow_forwardRoberto is a 78-year-old male who has recently been diagnosed with Class III Congestive Heart Failure (CHF). His physician told him that his left ventricle has become stiff and cannot relax. Although there is no current treatment that can “fix” Roberto’s condition, he is prescribed two medications: an ACE inhibitor and beta-blockers. Further testing reveals that Roberto has hypertrophy of the left ventricle. How does this finding relate to his diagnosis?arrow_forwardA person is known to suffer coronary arteriosclerosis and suddenly complaint of chest pain, the hospital diagnosis was acute myocardial infarction ---- 6 hours later he died of cardiac arrhythmia. What is the cause of death? What is the mechanism of death? What is the manner of death?arrow_forward
- HQ3. Label the structures seen in an anterior view of the heart.arrow_forwardA 64-year-old female calls the office requesting an appointment. She tells you that recently she has noticed that her pulse rate is around 50 BPM. She says that she feels tired and that she has no energy. She is requesting an appointment. 1. How do you handle this call? 2. Recalling what you were taught about bradycardic heart rates, what would you expect to be the first diagnostic procedure to be performed after the initial vital signs assessment? 3. Referring to Table 10-1, what would you expect the initial treatment to be prescribed for this patient? 4. Again, referring to Table 10-1, should the sinus bradycardia progress to a slower rate and the diagnosis change to one of the forms of heart block, what treatment would you anticipate being prescribed? 5. Referring to the enrichment Pacemakers and Implantable Cardioverter Defibrillators, what is involved in the actual implantation of the device? 6. Again, referring to the enrichment Pacemakers and Implantable Cardioverter…arrow_forwardWhich of the following conditions can cause irregularities in the sound of the heart? Mild atherosclerosis of the coronary arteries. Myocardial infarction Mild mitral valve regurgitation. Moderate bradycardia.arrow_forward
- Explain the physiological mechanisms that drive the changes in heart rate between conditions. 1) lying down and sitting up, regular breathing. 2) Between sitting up, regular breathing and immediately after exercise. 3) Between seated inhalation and exhalation. 4) immediately after exercise, and after 60 seconds of recording/recovery.arrow_forwardWhat is the cardiac output of a heart when the heartbeat rate is 60 beat.min-1, the diastolic pressure in the aorta is 80 mm Hg, and the stroke volume is 80 mL.beat-1? A. 4,800 mm Hg.min-1 B. 6,400 mL.mm Hg-1 C. 4,800 mL.min-1 D. 1.33 mL.beat-1 E. 0.75 mL.min-1arrow_forwardAll of the following are correct about the isovolumetric contraction, except ____________. A. This phase of the cardiac cycle begins with the appearance of the QRS complex of the ECG, which represents atrial repolarization and ventricular depolarization B. It represents the time period between the closure of the AV valves and the opening of the aortic and pulmonic valves, ventricular pressure rises rapidly without a change in ventricular volume C. The rate of pressure increase in the ventricles is determined by the rate of contraction of the muscle fibers, which is determine by mechanisms governing excitation-contraction coupling D. Ventricular chamber geometry changes considerably as the heart becomes more spheroid in shape; circumference increases and atrial base-to-apex length increasesarrow_forward
