Module 12: Otorhinolaryngology - Report 5: Operative Report, Tube Removal Rationale Report Professional Services: 69424-50 (Removal, Tube, Ear, Middle) ICD-10-CM: T85.698A (Complication, prosthetic device or implant, mechanical, obstruction), H61.23 (Cerumen) RATIONALE: The patient had tubes placed in his ears and now has been experiencing complications of infection and inflammation. The tubes are blocked and no longer providing drainage from the middle ear. The service performed is the removal of ventilating tubes under anesthesia, 69424. The removal of tubes under local anesthesia is considered bundled with the insertion of the tubes when performed by the same physician. This is a bilateral procedure; therefore, modifier -50 is appended. Ear wax removal is not separately reported as it is included in the procedure, 69424. ICD-10-CM: The diagnoses are listed in the surgical Preoperative section of the report as obstructed PE (pressure equalizing) tubes. This is a complication and as such, would be coded as one. This would be a mechanical complication of specified type (tubes) and would be coded with T85.698A. The ear wax (cerumen) could also be reported, H61.23.
