1. What are the 2 most pressing issues facing those providing care for this family? 10 points
The two most pressing issues illustrated in the case study is the decision to continue or withdraw care and the family’s ability to make this important decision in the midst of experiencing a crisis. The nurse acknowledges that the family is not fully capable of making a decision such as this as the mother states, “I see people talking to me, but I can’t hear what they are saying. It’s like my whole world has just fallen in on me.” It is apparent that a care conference should be held to discuss the issues at hand, however the physician was quick to suggest passing the buck to the facility at which the incident occurred. Patient centered care also means caring for the patient’s family and addressing any questions or concerns regarding the patient’s care if HIPAA is not violated. In addition, providing patient centered care involves the patient being an active participant in their care, and the nurse providing the patient with the tools to do so. Lastly, the nurse should advocate on the patient’s behalf and encourage the patient to advocate for themselves by asking questions and ensuring they fully understand treatment options. To provide this family with patient centered care, I would get to know the family by asking them what is important to them and what needs they feel they need addressed at this time. I would request a consult for support services from a chaplain or bereavement coordinator and allow the family time and privacy to continue performing their religious rituals. The family should be provided a comfort cart including food and refreshments, so the family will not have to leave the infants side for any reason until they are ready.
