This learning log will firstly describe what is meant by health and what determinants are thought to affect health. An individual’s health is influenced by a range of determinants. Health promotion plays a big part in an individual’s health and wellbeing, one type of health promotion can be the way in which education can influence an individual. This learning log seeks to address the role that health education and promotion can help patients to take more responsibility for their own health. Aside from health promotion, nurses and others in the care industry have to understand and adopt certain care approaches that take into consideration the individual needs of people and their families.
Health is very difficult to define as it is multidimensional and made up of different but linked elements. There is no universal definition of health as everyone has their own idea of what it means. However there has been a more rounded, summarised statement by the World health Organisation, Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity (WHO 1984). This statement suggests that health is a primary holistic concept. Nurses must have an open minded outlook onto the health of others. A nurse must see the patient as a whole and assess all aspects of health before being able to assist them in making decisions on how to make their life healthier. It focuses on individual behaviour towards a wide range of social and interventions. The three Health Promotion issues I have chosen to discuss are smoking, physical inactivity and unhealthy diet. According to NICE (2007) Smoking, poor diet and lack of exercise play an important role in the health and behaviour of people in relation to their social and cultural background. I have chosen to discuss a group of patients aged 70-75 years old who have previously been diagnosed with heart disease; multiple medical conditions include hypertension, high cholesterol and diabetes; they are currently being treated on a cardiac ward waiting for further tests. I will explore the three areas of Health Promotion and discuss how they are Linked to the patients underlining medical issues. Ethnics in nursing practice will be discussed, focusing on the patient’s views, values and principles; Inter-professional working in educating the patient and the impact it can have on how we think and react to our lifestyle choices; I will focus on Various models and discuss how they are linked in relation to the patient and health promotion. In accordance to the Nursing and Midwifery Council (NMC) (2015) full consent has been verbally granted; also in respect of their privacy the patients names and placement area will remain anonymous.

Nurses play an important role in promoting health within the patient, family, and community (Kemppainen, Tassavainen, & Turunen, 2012). The focus of patient care has been transferred from treating the illness to disease prevention (Mchugh, Robinson, & Chesters, 2010). The implementation of consultation, education, and follow up exams can increase the overall quality of life for an individual (Kemppainen et al., 2012). I will discuss the various roles of a nurse in health promotion along with the multiple work environments in which they can be implemented within. Often in practice, we as nurses deal with a variety of diseases and treatments and often have to react to the illness that the patient presents with upon our interaction. While this is an essential piece of our practice, we also have a duty to our patients to be proactive in preventing specific health-related consequences based on their risk factors and to promote their health and well being. The health promotion model is a theory of nursing that was developed by Dr. Nola Pender. Her theory was first proposed in 1982 and then revised in 1996. Dr. Pender was influenced by the work of James Hall, who studied people’s behavior and its correlation to health. Her work is a middle-range theory, as there are specific concepts that are observable, that she designed to be a counterpart to models of health protection in which health is simply viewed as the absence of illness. Pender’s theory was critiqued using the criteria from Fawcett (2005). Although there are limitations to Pender’s health promotion model, her theory is the best to use in nursing as it focuses on the promotion of individualized optimal health and produces increased positive outcomes.
