Case summary:
The staff nurse (Ms.T) is discussing about “wound care at home” with Mr and Mrs. L. The patient (Mr.L) tells the nurse that he is worried because he is “unable to go back to work on time”. When discussing, Ms.T noticed that the patient has trouble concentrating on what she was talking. He seemed to be concerned about his incision or wound. Mrs. L tells the nurse that “his office has been laying off workers” that why he is concerned.
Explanation of Solution
A diagnostic statement that contains three parts of the nursing process is known as PES format. The acronym “PES” that stands for “problem, etiology and signs or symptoms”. The clinical manifestations, signs and symptoms of the disease is known as defining characteristics.
The three pieces of assessment data that clusters together to form defining characteristics are given below:
- The patient is worried because he is suffering from a wound infection and unable to go back to work on time.
- The staff nurses noticed that the patient is having trouble with concentration.
- Mrs. L tells the nurse that Mr. L is anxious about his office (job) as it is laying off workers.
From these defining characteristics, Ms. T diagnoses that the patient has anxiety.
