Module 02 Assignment
- 3M Encoder and Coding Clinic Worksheet.
Overview:
Before attempting the
3M Encoder and Coding Clinic Worksheet
, you should read all the
information about 3M and Clintegrity Encoders in the Module 02 Lesson Content under the
section “Encoder Review”.
In your Coding Foundations Course, you created a personal account
with a personal password for the V Lab Medical Coder software where you will access the 3M
encoder.
If you did not save that personal password, you will need to contact the AHIMA help
line to find out your password.
If you never created the account, you will need to do that with
the access code sent to you in the weeks BEFORE the quarter in which you took Coding
Foundations.
If you can’t find that email, you will need to contact the Personal Support Center
and inform them that you did not receive the V Lab access code email and request a code.
If
they say they already sent you a code, request that they resend the email.
If you tested out of Coding Foundations and did not receive an access code to set up an
AHIMA VLab Account, please contact the Personal Support Center to obtain a V Lab access
code to set up your personal account with access to the 3M Encoder through the AHIMA
website. When you receive the V Lab access code from the University Bookstore, the email
will include instructions for redeeming the code for a personal account.
Once you have accessed the 3M Encoder and you are prepared to do screenshots, go to Part 1
of this worksheet and follow the directions for the three screenshots.
The instructions will take
you step by step as to how to navigate the 3M Encoder and find Coding Clinic.
Part 2 will take
you step by step through using Coding Clinic to research codes that are difficult to determine
the most accurate code.
Part 1 - Use 3M Encoder
Instructions
: Use the 3M Encoder and do the following:
1. In the 3M Encoder, on the Patient Information screen, do the following:
a.
Gender
: Set to female or use the drop-down box to choose male.
o
Remember: Gender is “Female” by default, but you can change it to “Male”
when coding.
b.
Age at Admission
: Enter 30.
c.
Product:
The default will be DRG Finder which is good for this assignment.
d.
Take a screenshot and paste it in the space below this sentence.
2. In the 3M Encoder, from the screen in the prior question, do the following:
a. Click the “Continue” bar and then choose “home” for
discharge status
.
Then press the
“Enter” bar to bypass “Admit Diagnosis,” which will take you to the screen where you
can begin to code. (A reminder that if you want to code inpatient you allow the
default of DRG Finder.
If you want to code for outpatient, you set the “product” on
the information page to APC Finder.) You will not be coding for this assignment, only
using the resource of the AHA Coding Clinic
.
This section is just to review how to get
to the coding screen so you can use the 3M Encoder for coding if you choose.
b. Click the icon of yellow stacked books on the right of the toolbar at the top of the
screen.
That takes you to all the resources available in this encoder.
c. Take a screenshot and paste it in the space below this sentence
.
3. In the 3M Encoder, from the screen in the prior question, do the following:
a. Click the link for “AHA Coding Clinic for ICD-9-CM and ICD -10 CM/PCS.”
You will now
be in the AHA Coding Clinic, so you are ready for Part II.
b. Take a screenshot and paste it in the space below this sentence

Question:
There appears to be a discrepancy in the Alphabetic Index for
aspiration bronchitis. When referencing "bronchitis" with subentry
"aspiration," the Index refers coding professionals to code J68.0,
Bronchitis and pneumonitis due to chemicals, gases, fumes and
vapors. However, when the term "aspiration" is referenced with the
subentry "bronchitis," the Index refers coding professionals to code
J69.0, Pneumonitis due to inhalation of food and vomit. What is the
appropriate code assignment for aspiration bronchitis, not further
specified?
Answer:
According to clinicians, aspiration bronchitis is more commonly caused
by aspiration of food rather than aspiration of gases. Therefore, code
J69.0, Pneumonitis due to inhalation of food and vomit, is a more
appropriate code assignment than code J68.0. The Centers for
Disease Control and Prevention, National Center for Health Statistics
has agreed to address the inconsistencies in the Index to Diseases
through the ICD-10 Coordination and Maintenance process.
Post the Code Book Page Number for the code and any Instructional Notes in this
box.
PG. 968; Code also any associated foreign body in respiratory.

You are spending the summer as an assistant learning how to navigate on a large ship carrying freight across Lake Erie. One day, you and your ship are to travel across the lake a distance of 200 km traveling due north from yourorigin port to your destination port. Just as you leave your origin port, the navigation electronics go down. The captain continues sailing, claiming he can depend on his years of experience on the water as a guide. The engineers work on the navigation system while the ship continues to sail, and winds and waves push it off course. Eventually, enough of the navigation system comes back up to tell you your location. The system tells you that your current position is 50.0 km north of the origin port and 25.0 km east of the port. The captain is a little embarrassed that his ship is sofar off course and barks an order to you to tell him immediately what heading he should set from your current position to the destination port. Give him an appropriate heading angle.

You are a biomedical engineer working for a small orthopaedic firm that fabricates rectangular shaped fracture
fixation plates from titanium alloy (model = "Ti Fix-It") materials. A recent clinical report documents some problems with the plates
implanted into fractured limbs. Specifically, some plates have become permanently bent while patients are in rehab and doing partial
weight bearing activities.
Your boss asks you to review the technical report that was generated by the previous test engineer (whose job you now have!) and used to
verify the design. The brief report states the following... "Ti Fix-It plates were manufactured from Ti-6Al-4V (grade 5) and machined into
solid 150 mm long beams with a 4 mm thick and 15 mm wide cross section. Each Ti Fix-It plate was loaded in equilibrium in a 4-point bending
test (set-up configuration is provided in drawing below), with an applied load of 1000N. The maximum stress in this set-up was less than the
yield stress for the Ti-6Al-4V…

The Weather Monitor. Your South American expedition splits into two groups: one that stays at home base, and yours that goes off to
set up a sensor that will monitor precipitation, temperature, and sunlight through the upcoming winter. The sensor must link up to a
central communications system at base camp that simultaneously uploads the data from numerous sensors to a satellite. In order to
set up and calibrate the sensor, you will have to communicate with base camp to give them specific location information.
Unfortunately, the group's communication and navigation equipment has dwindled to walkie-talkies and a compass due to a river-raft
mishap, which means your group must not exceed the range of the walkie-talkies (3.0 miles). However, you do have a laser rangefinder
to help you measure distances as you navigate with the compass. After a few hours of hiking, you find the perfect plateau on which to
mount the sensor. You have carefully mapped your path from base camp around lakes and…
