Chapter 10.5, Problem 2RQ

It's estimated that more than 7 000 hospital patients die each year because of drug errors, and many others suffer ill effects from being given the wrong drug or the wrong dosage Some hospitals are using bar codes attached to patients’ wristbands that allow hospital personnel who administer drugs to patients to electronically check to make sure the drug and dosage are appropriate Before administering a drug the doctor or nurse scans the bar code attached to the patient to see what drug is needed and when, and then the drug’s bar code is scanned to verity that the mediation is correct

But bar codes are not foolproof as a recent study of hospitals showed Nurses may develop a workaround that involves using photo copies of a group of patients bar codes which are then used to obtain drugs for then used to obtain drugs for the entire group. The nurse would then have a tray that may contain drugs of different dosages intended for different patients. At that point the bar code protection has been circumvented.

What action would you suggest to avoid the problem of workarounds?

It may happen that no response is received at all. If no response is received after 48 h, or if an order cancelation is received, the supplier will cancel the order. If an order confirmation is received within 48 h, the supplier will process the order normally. If an order change is received within 48 h, the supplier will update the order and ask again the retailer for confirmation. The retailer is allowed to change an order at most three times. Afterwards, the supplier will automatically cancel the order.
